Age Difference in the Trajectory of Medical Service Utilization, Medical Expenses and Health Outcome among the Elderly: A Latent Growth Model Analysis

Abstract

|
This study was to investigate the developmental trajectories of medical service utilization, medical expenses and health outcome of elderly people over 65, to identify the longitudinal relationship, to examine the determinants which influence
to the developmental trajectories, and to investigate the differences of these three factors and their relationships young-old and old-old age groups. Data were drawn from the Korea Health Panel and the study sample consisted of 2,276 subjects over 65 who participated in the survey from 2009 to 2013. Latent Growth Model (LGM) and Multi-Group Analysis was performed. The utilization of inpatient services and medical expenditure increased and the perceived health status declined over time. The major determinants on the developmental trajectories of inpatient service utilization, medical expenditure and health status were the needs factors of Andersen’s health behavior model, which were disabled, chronic disease and pain and/or discomfort. In the young-old group, the individuals on the high score of intercept in inpatient service utilization and perceived health status had lower rate of incremental changes of these factors, which mean the gaps of these factors between individuals had been lessened over time. However, the gaps were greater for the old-old age group.
